Steel's triple pelvic osteotomy in the treatment of acetabular dysplasia

Özet

Arthrosis is inevitable in patients with acetabular dysplasia. A pelvic osteotomy can prevent secondary degenerative changes in these hips. We have carried out a retrospective review of 12 hips after triple osteotomy with an average follow-up of 6.5 years (5 to 8). In 92% of the patients there was pain improvement, but 42% continued to limp. Radiographic assessment showed improvement of the average centre-edge angle by 24.6 degrees, anterior centre-edge angle by 26.25 degrees and correction of the acetabular index by 16.3 degrees. The degree of osteorthritis improved in six hips over a period of six years. We observed superficial infection in two patients. In two patients the correction was lost because of insufficient osteosynthesis and additional surgery was performed for reosteosynthesis. In general triple osteotomy can achieve enough correction and coverage. This type of osteotomy is easier than other periacetabular osteotomies. However, it has disadvantages such as acetabular lateralization, pelvic asymmetry and difficulties in correcting the acetabular coverage and stable osteotomy fixation.
