Objective: Primer hyperparathyroidism (PHPT) is defined as a condition of hypercalcemia
due to parathyroid glands’ hyperfunction. PHPT cases are caused by 80% to 85% of Parathyroid
adenomas and 1% of parathyroid malignency. Despite being a most important sign
of hyperparatiroidism, PTH can also use for treatment. PTH can be used in oral surgery for
treatment of MRONJ(Medication Related Osteonecrosis of Jaw), osteoradionecrosis defects
and brown tumor, and also to enhance consolidation phase of distraction osteogenesis.
Case: In following case patient referred our clinic with complaint of mandibular pain, and
after radiographic evaluation bilateral multilocular radiolucent lesions was noticed. Because
of bilateral involvement, patient looked for a systemic problem such as primer hyperparathyroidism
so before incisional biopsy, patient was advised blood investigations that revealed
PTH level of 784 IU/L. Patient immediately directed to endocrinology. As a consequence of
other blood and radiographic investigations, patient diagnosed Hurthle Cell Follicular Carcinoma
and thyroidectomy operation was done by endocrinology department.
Conclusion: Evaluation of radiolucent lesions on jaws may act a significant part of detecting
endocrine malignancies. Oral surgeons’ role in this issue is quite important and crucial for
early diagnosis.
